How Pilates addresses the physical and psychological symptoms of perimenopause and menopause.

Pilates is one of the most frequently recommended exercise approaches for people with osteoporosis and osteopenia — and one of the most frequently misapplied. The method's emphasis on controlled movement and spinal alignment makes it naturally well-suited to bone health. But several classical Pilates exercises pose genuine fracture risk for people with low bone density. This guide explains the evidence, the exercises that help, and the critical modifications that make Pilates safe.

Bone is living tissue that responds to mechanical load. When muscles pull on bones through their attachment points, the resulting stress stimulates osteoblast activity — new bone formation. This is Wolff's Law: bone adapts to the loads placed upon it. Conversely, without adequate load (sedentary behaviour, bed rest, weightlessness), bone density declines.
Osteoporosis — defined as a T-score of -2.5 or lower on DEXA scan — represents a significant reduction in bone mineral density. It is most common in postmenopausal women (due to oestrogen loss) and in older men, but can affect any age group. Osteopenia (T-score -1.0 to -2.5) is a precursor state that responds well to exercise intervention.
The goal of exercise for osteoporosis is twofold: stimulate bone formation through safe mechanical loading, and reduce fracture risk by improving balance, coordination, and postural muscle strength. Pilates addresses both — when appropriately modified. The challenge is that the classical Pilates repertoire includes exercises that compress the spine in flexion, which can trigger vertebral compression fractures in people with low bone density. Understanding this distinction is the critical starting point.
These exercises are generally safe and beneficial for people with osteopenia and osteoporosis. They maintain the spine in neutral or extension (reducing vertebral compression risk), load the hip and spine through safe ranges, and develop the postural muscle strength that reduces fall risk.
Back extension work (swan prep, swimming)
Extension exercises load the posterior vertebrae and paraspinal muscles in a bone-stimulating way without the dangerous compressive flexion forces of forward-bending exercises. Swan prep — lying prone, lifting the chest — is particularly valuable. Swimming (alternating arm-leg lifts prone) develops the extensor endurance that maintains upright posture and reduces the thoracic kyphosis that worsens with osteoporosis.
Standing balance and hip strength work
Falls are the primary cause of osteoporotic fractures. Improving single-leg balance, hip stability, and reaction speed is therefore one of the most important exercise goals for people with osteoporosis. Pilates standing series — including hip circles, single-leg exercises with wall support, and heel raises — directly targets these outcomes.
Bridge and glute strengthening
The bridge exercise loads the hip extensors and lumbar spine extensors in a way that is safe for most osteoporosis presentations (spine in neutral throughout, not flexed under load). Progressive glute strengthening is particularly important for hip fracture prevention — hip fractures are the most catastrophic consequence of falls in people with osteoporosis.
Reformer footwork series
The reformer footwork series — pushing through the feet against spring resistance while lying supine — loads the tibia, femur, and hip in a controlled, low-impact way that stimulates bone formation. The spine is supported in neutral on the carriage throughout, making this one of the most appropriate bone-loading exercises for people with significant osteoporosis.
Side-lying leg series
Hip abduction, external rotation, and hip circle exercises in side-lying load the femoral neck — the most common site of osteoporotic hip fracture — through the abductor muscles. This is one of the most direct interventions for hip bone health available in the Pilates repertoire.
The following classical Pilates exercises are contraindicated or require significant modification for people with osteoporosis. The primary risk is vertebral compression fracture from loaded spinal flexion — the vertebrae are most vulnerable when the spine is bent forward under load.
A 2021 systematic review and meta-analysis published in Osteoporosis International analysed 12 randomised controlled trials examining Pilates exercise in postmenopausal women with low bone density. The authors found statistically significant improvements in lumbar spine bone mineral density (BMD) and femoral neck BMD compared to control groups, along with significant improvements in balance and functional mobility — both of which are critical fall-risk factors.
The effect on BMD was modest — roughly 1–3% improvement compared to control groups — but clinically meaningful. A 2–3% improvement in femoral neck BMD over 6 months represents a meaningful reduction in fracture risk. More significant are the improvements in balance and muscle strength, which reduce fall probability independently of bone density.
The National Osteoporosis Foundation (US) and the Royal Osteoporosis Society (UK) both include exercise as a first-line recommendation for osteoporosis management. The Royal Osteoporosis Society's guidance specifically recommends resistance and balance exercises — the core elements of Pilates — as appropriate for people with osteoporosis when correctly supervised.
Can Pilates increase bone density?
Research suggests that weight-bearing and resistance exercise, including Pilates, can slow bone density loss and in some cases produce modest increases. A 2021 systematic review in Osteoporosis International found that Pilates-based exercise programmes significantly improved bone mineral density at the lumbar spine and femoral neck in postmenopausal women compared to control groups. The effect is not as large as high-impact exercise (jumping, running), but Pilates is safer for those who already have significant bone loss.
Is Pilates safe with severe osteoporosis?
Pilates can be safe with severe osteoporosis if the exercises are appropriately modified and supervised by a qualified instructor familiar with the condition. The critical modifications are: no spinal flexion under load, no spinal rotation combined with flexion, and no high-impact movements. Many of the classical Pilates mat exercises require these modifications for people with T-scores below -2.5 (severe osteoporosis). Always inform your instructor of your diagnosis and provide your most recent DEXA scan result.
What Pilates exercises are contraindicated for osteoporosis?
The primary contraindications are exercises involving flexion of the spine under load — particularly the roll-up, the hundred in full position, double-leg stretch, and spine stretch forward in seated. Rotational exercises combined with flexion (saw, corkscrew) are also contraindicated. Exercises that are generally safe include: back extension work (swan prep, swimming), hip strengthening (clam, bridge), standing exercises with support, and gentle reformer footwork.
Is reformer Pilates better than mat for osteoporosis?
Reformer Pilates has advantages for osteoporosis management because the spring system provides resistance (bone-building stimulus) while the padded carriage supports the spine in safe positions. The long box series, footwork, and standing exercises on the reformer are particularly appropriate. However, mat Pilates — suitably modified — is equally valuable for the extension and stability work that benefits bone health. One-on-one reformer sessions with a clinically-trained instructor are the gold standard starting point.
